At Hazelbrook Care Home in Leicester, we work in close partnership with health and social care professionals to ensure safe, timely and well‑coordinated transitions into our specialist care services. We understand the importance of seamless handovers from hospital, community teams or commissioners, and are experienced in supporting individuals with complex needs ranging from dementia and mental health to neurological and high‑dependency nursing care.

Our skilled multidisciplinary team, including registered nurses, clinical care practitioners and therapeutic support staff, collaborates with social workers, hospital discharge teams, case managers, NHS Continuing Healthcare assessors and local authority professionals to deliver consistent, person‑centred care from the very start.

Making a Referral

Hazelbrook welcomes referrals from healthcare professionals who are responsible for planning safe, supportive care pathways for their service users. We offer a flexible and responsive approach to both longer‑term placements and shorter‑term transitions, including step‑down care following hospital discharge where appropriate.

To begin the referral process, professionals are encouraged to provide detailed clinical and social information so that we can assess suitability effectively. Once a referral is received, our Clinical Assessors will liaise with you promptly to clarify any details and determine the next steps. Assessments may be conducted in person, virtually or, where required, within the current care setting. This collaborative process helps ensure a smooth, clinically sound transition into our services.

Specialist Dementia Nursing Care

For individuals living with dementia, including those experiencing changes in memory, behaviour or emotional response, Hazelbrook offers specialist dementia support built around reassurance, familiarity and structure. Our team has specialist training in dementia awareness and behavioural understanding, allowing us to respond with confidence, patience and empathy. Rather than simply responding to symptoms, we help residents find comfort in familiar routines, calming surroundings and activities tailored to their interests and needs.

Complex Nursing Care & Physical Health Support

After receiving a referral, our Clinical Assessor will review the details and work with you to clarify clinical needs and priorities. An initial assessment is scheduled promptly in the context of the resident’s health and social circumstances, and a transition plan is agreed upon between all partners.

We maintain open channels with all involved professionals, sharing updates and engaging in regular reviews. Our team liaises directly with GPs, specialists, community services and others as needed to ensure joined‑up care.
